Neonatal outreach team
The neonatal outreach team support you and your baby as you go home and provide continuing support at home once your baby has been discharged.
The neonatal outreach team has specialist neonatal nurses who will meet with you during your baby’s stay on the neonatal unit and together identify and agree a plan of care to enable you to go home as soon as possible.
The number of visits will depend on your family’s individual needs. We may visit you and your baby for up to 6 weeks after you go home and offer 1 to 2 visits per week. We aim to contact you within 48 hours of you going home either by telephone or by a pre-arranged visit.
If we have missed you being at home, please contact the Neonatal Outreach Team to rearrange another visit.
What does the neonatal outreach team offer?
During the visits the team will monitor your baby's feeding, growth and development.
If needed, we can:
- support you with breast feeding
- review your baby’s medication
- take any blood samples
- offer nursing support if your baby needs to have oxygen at home.
The team will also offer you advice and guidance in caring for your baby and can answer any questions which you may have.
The team will work with other professionals involved in your baby’s care and will tell them if there are anything that may need attention or treatment.
We try to be on time for visits. As we cover a large area, please be understanding if we arrive later than expected. We will try to contact you if we are going to be late. Sometimes we may need to cancel or rearrange visits at short notice. We will contact you to rearrange your appointment, if necessary.
How can I contact the neonatal outreach team?
The neonatal outreach team can be contacted Monday to Friday (except bank holidays) between 9am and 5pm on 07909 848023 or 07909 811709.
If the team are unavailable to take your call please call 01709 424488 or 01709 424629 and a member of the neonatal unit team will take your message and pass this on to us.
If your baby is unwell and you need urgent help, call 999 or take your baby to the Urgent and Emergency Care Centre (UECC).
